The National Committee for Quality Assurance (NCQA) reviewed the submission, and the practice has received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long term, participative relationships.

About Danish Siddiq, MD
Dr. Siddiq is board certified in infectious disease and board certified in internal medicine. He comes to Access Health Care from Baylor College of Medicine where he completed a fellowship in infectious diseases and was award the position of Chief Fellow. Dr. Siddiq also holds an additional certificate of qualification from the Baylor College of Medicine & University of Texas School of Public in Clinical Scientist Training program. He completed his internal medicine residency also at Baylor College and received his Bachelor of Medicine Bachelor of Surgery (MBBS) from the prestigious Aga Khan University in Pakistan. Dr. Siddiq is a member of the Infectious Diseases Society of America, the American Medical Association and the American College of Physicians.
