Harish M. Madnani, MD, and His Team Receive Renewal of Recognition Status for Their Patient Centered Medical Home (PCMH)

Madnani, MD, board-certified in internal medicine and hospice and palliative medicine, and his team have achieved renewal recognition under the National Committee for Quality Assurance PCMH Recognition Program.

The National Committee for Quality Assurance (NCQA) reviewed the submission, and the practice has received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long term, participative relationships.

 

Earning PCMH recognition is a distinction in which a practice can take pride. The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduced cost. Medical Homes foster ongoing partnership between patients and their personal clinicians, instead of approaching care as a sum of episodic office visits. About Harish M. Madnani, MD

Dr. Madnani is board certified in internal medicine, board-certified in hospice and palliative care medicine, and specializes in geriatric medicine and clinical hypertension. He earned his medical degree in 1978 from SMS Medical College in Jaipur, India. Dr. Madnani completed his surgical internship at Flushing Hospital Medical Center in Flushing, New York. He completed his residency in internal medicine at St. Barnabas Medical Center in Livingston, New Jersey, where he was awarded the position of Chief Resident during his final years of training. He was responsible for directly supervising several junior residents and interns.
