The National Committee for Quality Assurance (NCQA) reviewed the submission, and the practice has received NCQA Patient-Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long term, participative relationships.
Earning PCMH recognition is a distinction in which a practice can take pride. Dr. Patel is a family practitioner and comes with a wide range of disciplines and stresses preventative medicine and health maintenance as a core philosophy. Dr. Patel received her medical degree from the Medical University of Debrecen. She completed residency in family medicine at Southside Hospital North Shore on Long Island, New York.Â
