Ana C. Cruz-Diaz, MD, and Her Team Receive Recognition Status for Their Patient Centered Medical Home (PCMH)

Citrus & Hernando Counties, Florida (February 6, 2020) – Access Health Care Physicians®, LLC, is proud to announce that Ana C. Cruz-Diaz, MD, primary care physician, and her team have achieved recognition under the National Committee for Quality Assurance PCMH Recognition Program.

The National Committee for Quality Assurance will list Dr. Cruz-Diaz and her location on its website with a recognition seal. Dr. Cruz-Diaz and her team practice at 7729 E. Pine Lake Lane, Floral City, Florida 34436. Call 352-765-3003 to schedule an appointment. New patient's are always welcome.

The National Committee for Quality Assurance (NCQA) reviewed the submission, and the practice has received NCQA Patient Centered Medical Home (PCMH) Recognition for using evidence-based, patient-centered processes that focus on highly coordinated care and long term, participative relationships.

Earning PCMH recognition is a distinction in which a practice can take pride. The NCQA Patient Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patient's experience of care and reduced cost. Medical Homes foster ongoing partnership between patients and their personal clinicians, instead of approaching care as a sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the healthcare system. Research shows that Medical Homes can lead to higher quality and lower costs and can improve patient and provider reported experiences of care.

About Ana C. Cruz-Diaz, MD

Dr. Ana C. Cruz-Diaz is a general practice physician who comes to Access Health Care from Puerto Rico where she served as a physician in general practice in Canovanas, Puerto Rico, where she practiced for 13 years. She treated a wide variety of illnesses to patients from 18 through geriatrics. Dr. Cruz-Diaz is accustomed to performing a variety of primary care services, wellness checks, pap tests, EKG, minor procedures and HEDIS prevention measures. She is also recognized as a diabetes educator.

She completed her rotational internship at Hospital HIMA San Pueblo in Caguas, Puerto Rico. She received her Doctor of Medicine degree from Universidad Autónoma de Guadalajara, Mexico.
